ALS Northwest is fortunate to be able to offer financial assistance to people living with ALS in Oregon and the lower six counties of Southwest Washington. Like our other programs and services, the grant program is funded by community support and the chapter's annual fundraising events such as the Walk to Defeat ALS.

There are two separate grant periods offered to all people living with ALS in our region every year. The maximum reimbursement amount for one grant period is $500. This allows us to potentially reimburse up to $1,000 of qualified ALS-related expenses for each person living with ALS in our area in a single fiscal year (February 1- January 31). The first grant period runs from February to the end of July, and the second is from August to the end of January.

Chapter Grant Process

How to Apply

At the beginning of each new grant period, we will send people living with ALS in our area an email and a letter inviting them to apply, with a simple Chapter Grant Application form included. Once a completed application is received, the Chapter will respond to an applicant within 1-2 weeks to communicate their approval status. Once approved, we will send an approval notice, reimbursement forms, and a list of eligible expenses.

Reimbursement Requests

Once a person living with ALS has been approved for a Chapter Grant, they need to fill out the one-page Grant Reimbursement Form. If they are requesting reimbursement for eligible mileage expenses for travel to and from ALS medical appointments, they need to also fill out the Grant Mileage Form. If they are requesting reimbursement for respite care - care provided by someone other than your primary caregiver to give them some time off - they need to also fill out the Respite Reimbursement Form.

Lastly, awardees must attach receipts for any and all eligible ALS-related purchases and expenses. Review Chapter Grant Eligible Expenses list before submitting. All receipts must be dated within the current grant cycle. Please refer to the Chapter Grant Reimbursement form for specific time frames.

Reimbursement requests can only be sent in by individuals who have received a chapter grant approval for the current grant cycle. If you need a reimbursement request form, please contact us at careservices@alsoregon.org.

Once we have received reimbursement paperwork, we will mail awardees a check within 1-2 weeks of receipt. Awardees may submit multiple reimbursement forms and receipts until they have reached the $500 total or can submit all $500 worth of eligible expenses at once.
