Chapter Grants Program

Our August 2022 - January 2023 grant period is currently closed. Please check back here on February 1, 2023 to apply for our next chapter grant cycle. 

Our chapter is fortunate to be able to offer a grant program! Like our other programs and services, the grant program is funded by community support and the chapter's annual fundraising events such as the Walk to Defeat ALS.

There are two separate grant periods every year. The maximum reimbursement amount for one grant period is $500. This allows us to potentially reimburse up to $1000 of qualified ALS-related expenses for each person living with ALS in our area in a single fiscal year. The first grant period runs from February to the end of July, and the second is from August to the end of January.

How to Apply

At the beginning of each new grant period, we will send people living with ALS in our area an email and a letter in the mail inviting you to apply, with a two-page Chapter Grant Application form included. Return the completed application to us, and we will reach back out to you within 1-2 weeks to communicate your approval status. If approved, we will send you an approval notice along with reimbursement forms and a list of eligible expenses.

Reimbursement Requests

Once you have been approved for a Chapter Grant, please fill out the one-page Grant Reimbursement Form. If you are requesting reimbursement for eligible mileage expenses for travel to and from ALS medical appointments, please also fill out the Grant Mileage Form. If you are requesting reimbursement for respite care - care provided by someone other than your primary caregiver to give them some time off - please also fill out the Respite Reimbursement Form.

Lastly, attach receipts for any and all eligible ALS-related purchases and expenses. Please review our Chapter Grant Eligible Expenses list before submitting your reimbursement request. All receipts must be dated within the current grant cycle. Please refer to the Chapter Grant Reimbursement form for specific time frames.

Once we receive your reimbursement paperwork, we will issue and mail you a check within 1-2 weeks of receipt. You may submit multiple reimbursement forms and receipts until you have reached the $500 total, or you can submit all $500 worth of eligible expenses at once.

Frequently Asked Questions

Q: I was approved for the second period Chapter Grant (August to January), but I have a receipt from an expense incurred in April. Can I submit that receipt for reimbursement?
A: No. All receipts must be dated within the current Chapter Grant period. Please refer to the Chapter Grant Reimbursement form for specific time frames.

Q: Can my spouse or caregiver fill out the application and/or reimbursement paperwork for me?
A: Absolutely!

Q: Can I fill out paper copies of the forms and submit images of them?
A: Yes! We understand it may not be convenient or possible for you to fill out the PDFs electronically, or physically mail them to us. You are more than welcome to send us images of the completed forms.

Q: I was approved for a Chapter Grant, but forgot to request reimbursement. Am I still eligible to apply for future Chapter Grant periods?
A: Yes!

Q: I am a spouse of a person living with ALS. Can I be reimbursed for expenses related to their disease?
A: Payment will only be made to the person living with ALS.

Q: Is there a deadline for submitting a grant application?
A: Yes. Grant applications must be received by either January 15th or July 15th (for corresponding grant periods) to be considered.

Q: What if I miss the grant application deadline?
A: We encourage you to apply again the next cycle. Each cycle runs February 1 - July 31 and August 1 - January 31 each year.
